I run on-line maths diagnosis in Moodle, but it is designed for, and integrated into our assessment for our course. I have my own randomised question banks but they are purely for support maths for chemistry/biochemistry. We do hope to make them available to use or for adaptation (i.e. the PHP files through which we create the random question banks) and I will be presenting this at the APT conference in Greenwich in July.
Numbas might be worth a look though and you can write your own questions....but requirements for maths vary greatly with discipline. In biosciences, there are 2 additional problem associated with numeracy -interpreting and solving word problems and maths phobia.
In terms of writing skills, I find these out when students do their first essay!
