I'd also be interested to hear from any institutions currently
monitoring these areas. ECU's project on staff disclosure is looking at
sexual orientation (in addition to ethnicity and disability) and will be
publishing a report in the summer, in which we hope to include some
examples of good practice. Further details can be read at

Dear Nona & Kez,
We use a monitoring form (attached) I hope that is of use? It includes
questions referring to sexual orientation and religion/belief.
Kind regards,

Staff
Hello Colleagues,
Happy New Year.
We are currently developing work around monitoring of staff around
sexual orientation and religion and belief. I was wondering if anyone
has had involvement of this and is willing to share their
approach/experiences off line, any help, advice, guidance will be
appreciated.
Many thanks
