I am currently drafting some forms to support our Dignity at Work and Study
policy.
One form is for Bullying and Harassment Advisors to fill in with the individual
and includes equalities monitoring and general questions to help us monitor the
service (how many meetings have you had with this person?, what was the
nature of the issue e.g. sexual harassment). This information will be
anonymous.
The second is for the member of staff/student to fill in about their experience
of meeting with a Bullying and Harassment Advisor and asks questions
evaluating the service e.g. was the information given helpful?, how did you
find out about the service?.
Does anyone else have similar or different forms to record issues of bullying
and harassment at this initial stage? If you wouldn't mind sharing these, please
