Like Katya, I understood the best way of collecting data is to have one box 'married/civil partner' so that respondents are not obliged, in answering the question, to reveal his/her sexual orientation.
I also have misgivings re using 'indeterminate' in a self-identifying form. I can see it may be appropriate when returning a statistical count, say to HESA to record the number of staff for which data on whether they are male or female has not been recorded. However it seems really inappropriate to ask individuals if their gender/sex is indeterminate. I hope HESA will instead be encouraging/enabling HEIs instead to use appropriate questions re gender/gender identity as e.g. in the recent ECU guidance on supporting trans staff and students.
Also in ethnicity classification, the 'Other' category which usually follows after Chinese is not as far as I know meant as 'Chinese Other. Any thoughts?

Many thanks for sharing this - it's really useful to see how people approach explaining the need
for data collection, and I like the fact you've included a commitment to taking action to address
adverse impact.
I wonder about the marital status box, though - what are people doing about civil partnership
status? It would be false for someone in a civil partnership to describe themselves as 'married',
and very misleading to describe themselves as 'not married'! And there was an article in the
Guardian last December pointing out that listing marriage and civil partnership as separate
categories actually forces people to out themselves - a report from the CAB recommended a single
'married/civil partner' category. ('Forms pose discrimination risk' Tuesday 4 December 2007)
Any thoughts?

we have just revamped our form for staff as per the new HESA disability
categories. It's attached.
We also changed the gender question re HESA
We extended the form to explain why we collect the data and included
some information from ACAS.
