The organization I work for uses a QuILTs system (Quality Improvement Learning Teams) which is really all about peer and self assessment, reflection, sharing best teaching / learning practices, encouraging collaboration and support from colleagues....that sort of thing. Works quite well, and echoes some of what Cambridge teach on their DELTA course (reflection-action).
We also have 'walk-throughs' and 'desk audits', all based on simple criteria. It gives me a clear idea about what my employer is looking for, next steps (and what hoops to jump through) etc.
No system I've seen is perfect, but this seems to work well (so far).
