I'm not sure if my kids have NINOs (but they do have NHS numbers).
The E111 covered "any insured person and members of his/her family". It
isn't clear, in retrospect, what "Insured" means here. Having been
issued with a NINO, perhaps - although the form only asked for an NHS
number. Does having an NHS number mean one is "insured"?

There may have been some sort of "consolidated" application form for the
E111's, but I don't recall it. Each member of the family has their own
E111, and as far as I remember they were filled in separately by hand
and stamped, without a "family" form as mentioned below. But perhaps I'm
simply mis-remembering.

I've found a copy of the "family" form here:

And it has a box for the head of household to certify that the people
listed have consented to disclosing their details (by implication, to
the PPA). I suppose it's difficult for the HoH to fill in the form
without them agreeing to disclose the details to him as well.
