Having been issued with a NINO, perhaps - although the form only asked for an NHS number. There may have been some sort of "consolidated" application form for the E111's, but I don't recall it. Each member of the family has their own E111, and as far as I remember they were filled in separately by hand and stamped, without a "family" form as mentioned below.
