Why not use the Purpose form at the end of the Notification Handbook
There is nothing to say that it can't be "developed" in the way you want

I actually spoke to someone in the Notification Department at the DPC's
Office to ask them to send me some blank forms but apparently they are not
issuing them. What they are doing is allocating a notification number as
soon as you fill one in. I too would prefer to have some blank ones
