I don’t have dealings with medical records but (apart from right to see your records under the DPA) it seems reasonable that individuals have a right to know who has accessed their medical file under privacy/confidentiality grounds, perhaps? Certainly, information that identifies a professional (doctor, nurse etc) carrying out official duties (including access to patient files, adding/editing content etc.) wouldn't normally be withheld.
Happy to hear other views though.
