I guess Guide Dogs for the Blind Assoc. may be as good a place as any to start with. They set up similar contracts with schools across the country and I expect the physical/sensory requirements of any contract would be similar in content.

Good afternoon all
I am currently writing a contract to secure mobility training for visually
impaired students. The contract is between a co-ordinating organisation who
will recruit, manage and provide the service on our behalf.
In an attempt to not reinvent the wheel and to learn from what others have
done, I would be very grateful if anybody who had done a similar
service-level agreement might be prepared to share this with me. I have
searched the archives but haven't found anything - please tell me if I have
missed something.
