I am not aware of a standard form, but would suggest that you contact your local Public Health Laboratory Service or Health Authority for a list of notifiable diseases. There is a statutory requirement to disclose a notifiable disease.

Is there a standard procedure (equivalent to s29 for police) for releasing data to health service personnel on the grounds of safeguarding public health?
I haven't come across this before!
