This is a common problem as in files the contact details etc of family members will be present - I would normally apply reasonableness where possible, however it is a fair point that 'one cannot be certain of the 'friendliness' of any family relationship.
In the ICO guidance, when responding to SAR's they say we are able to release third party information firstly: with consent, secondly, without consent but if it is reasonable to disclose and we should also pay attention what the requestor already knows. Therefore would this apply, say to minutes of a conference, wherby the inidividual has attended, however there is information discussed about third parties.
I know it is possible to overthink these requests, the problems often come when you redact something, then realise later on from within the informaiton held, that the information is already known to the individual making the request.
