My guess - it's human error. However, this might not be the end of the
story. Has the person been trained in DP / Confidentiality? Is there a
better way of getting the data off the system (i.e. should they be
printing labels from the systems rather than copying it?).
I could keep going - the real question is whether the Data Controller
has taken sufficient steps to prevent the breach. The ICO sometimes acts
as if human error is always preventable, and this is plainly wrong. If
the organisation can demonstrate appropriate measures, it has complied
even if mistakes get made.
