If the evidence (as is so in this case) is from someone who has the following credentials:
A qualified Psychologist.
A Full Member of the Association of Educational Psychologists.
Employed as an Educational Psychologist by an LEA.
What could possibly be sub-standard with the report and rejected by the Access Centre Assessor?

I totally agree with Bryan and Faye.
From an Advisers point of view, there is nothing worse when students have had an external assessment done, with substandard evidence, and then been turned down for the recommendations. We are then left with an annoyed student who requires further evidence to support the recommendations.
It is far easier, and makes a lot more scence to support the student to get the right evidence in the first place, and therefore meaning the have a smooth transit through the assessment process.
Evidence which secures a Needs assessment, and that which supports recommdations can be wildly different, and it is important that an assessor can make their recommendations with as much background knowledge as to the students difficulties as possible., otherwise can they really justify the recommendations at all?

No, not a first at all, just the Assessor acting in a professional manner. The alternative, which can be acceptable to LAs - as the LA has agreed that the student is eligible for DSA - would be for the Assessor to undertake the assessment and conclude with no recommendations for support there being no grounds for such. Which is better? I would also hope a Disability Officer would not organise additional exam provision with inadequate or none existent supporting evidence either.

We have it that a Access Centre has refused to carry out a needs assessment on the grounds that the evidence produced is not sufficient/below standard - even though it has been accepted by the Student's LEA Award Officer.
Is this a first?
