Being devils advocate here:- What about where the sickness record is for
stress related illness and that illness has developed into something more
serious which recurring reference back to earlier signs of the illness is
traumatic for the individual?
How would that be recognised without reference to the appropriate health
professional?
Is it possible to identify, or should people be willing to make decisions
about, the current medical state of an individual and the impact any minor
sounding illness of the past might have on them?

seen, there would be no need to obtain any medical opinion, but if the
medical data had been provided by an occupational health physician, the
normal system of vetting for the "serious harm test" should take place.
