Dear Claire et al,
I apply via Mary Holt, Manager, Education and Training, NHS Executive,
Department of Health, 40 Eastbourne Terrace, London, W2 3QR. Tel: (0171) 725
5300.
Please note that it is not as easy as applying for DSA from LEAs. Three
quotations will be required (including assessment of need from Access
Centre). Be prepared for a protracted application procedure - but don't
quote me.
