We've converted out handbook for students with disabilities into HTML and
put in on the web, but admittedlty it may be difficult to find because the
URL is ridiculously long, due to the way our web site is structured. Have a
look at

(see what I mean!)
I also did a bit of a survey of our institution and put together 'assistive
technology at the University'.
Uni of Westminster have put links to many of the HE institutions disability
services home pages from the CCPD page.
