The more questions you ask the more threatening the form become to a
dyslexic student. I'm not suggesting that your forms are as bad, but the
NHS free treatment form AG1 (from Dentists etc.) has enough verbage on it
to make even good students decide they would rather pay the fees than fill
in the form.

Dyslexic students (inc. those yet to be identified) are likely to lose one
half of the pair. This could mean that you never get the disability bit. I
always find one A3 sheet more user friendly than 2 by A4 etc. for this
reason.
A separately addressed 'nesting' envelope for a 'tear-off' disability
declaration is one solution to the confidentuality issue.
