A short but concise privacy statement, which also directs to where a full privacy notice can be read, would be ideal - and yes on every data capture form in print or online, directing individuals to it if you meet them and receive a business card (a link to your organisation's privacy notice on your own business card might also be good), email footers (see mine below) - really wherever you think you might be obtaining personal information from an individual directly.
And if you are not obtaining personal information from the subject directly, and they have not previously been provided with your privacy statement / notice (for instance new leads / prospects), providing them with this statement within 30 days of obtaining their personal information is a must.
What you include within this short statement really depends on what you are doing with their data - Yes to including information about anything that might be considered more intrusive, No to listing every right and freedom they have with regards to the data - that can be covered in the full privacy notice.
