I don't think that just asking "Do you have a disability?" is the question, then: the question should be "Do you claim a guaranteed interview on the grounds of disability?", because it's perfectly reasonable for someone to have a disability but not wish to avail themselves of the guarantee. And any question relating to appropriate arrangements should be contained in the invitation to interview. My view is that the question, or the compulsion to answer it, is wrong.
More trivially, I really resent the "Prefer not to say" answer - it sounds so prissy, and is only an artefact of dropdown lists and the need to be able to deselect an answer. I'd much rather the third answer was just a dash or "No answer", although I'd go with "None of your effing business" when dealing with certain organisations.
