I think Jon (and others) hit the nail by emphasising purpose - and as others have said you need to look at possible linkages.
On its own the answer is no.
But if this dataset is processed e.g. within a health service to recharge for cost recovery it would probably be processing sensitive pd , because it can be relinked to the treatment which says something about health.
If it is used to market dental services irrespective of identifies need it probably would not be - but other issues would arise!
