I think that as you suggest, the fact that A has provided information to B about C makes A's and no doubt C's identity disclosable to B subject to sections 7(4) and (5) of the DPA. If the personal data identifies an individual who has provided PD about the DS that identity is disclosable if (a) the individual consents (b) disclosure without consent is reasonable in the circumstances or (c) the individual is a health professional, social worker or teacher (plus some others) and is identified in the data subject's health, social work or education record respectively, in which case their identity cannot be withheld unless they would be at risk of serious harm. (The last bit comes from the various subject access modification orders).
