It depends on the situation.
I work in NI Civil Service and we have 12 Departments, each has their own structure with an information manager and SIRO.
If the data breach affects only 1 department then the info manager in that area usually looks into the breach and does the hard work.
Any recomendations etc would then be agreed by the SIRO and perhaps the Perm Secretary and maybe the minister.
Ultimately its not on the informatio manager.
If the breach affects more than 1 department then my team get involved in a co-ordination role and usually liaise between departments and perhaps a 3rd party supplier.
We also if required knowledge share across an information managers forum and a SIRO forum and if need be to Permanent secretaries.
We did have one breach that affected all 12 departments within the last year, so lots of views, comments and opinion.
Its never on the info manager to ensure things are done, but the work would fall to them, ultimate responsibility lies with the SIRO and/or Perm Secretary.

Good morning everyone,
Currently, in the local authority I work, data breaches are handled by yours truly in liaison with the relevant senior managment of the affected area. My role essentially consists of analysing the situation and advising on immediate and longer term actions that can/should be taken by the business in order to ensure that damage is mitigated and the chance of a repeat incident is minimised as far as possible. Basically I advise on what actions should be taken to maximise/ensure DPA compliance, among other things.
My question, to anyone who performs a similar function or oversees someone that does, is what level of responsibility does the colleague that handles data breaches possess when it comes to the advice they provide? Do they advise the business area directly or is their advice signed off/ratified by their line management before being given to the business area responsible for the incident?
Any and all answers gratefully received either on or off list.
