I don't understand why the penalty for a single unintentional error is
higher than the penalty for repeatedly making the same error even after it
has been raised at a senior level.
Given that encryption is unwieldy and may not always be appropriate or
possible, surely the minimum would be to create these documents with
passwords, and provide the password to the recipient by a different channel
(phone or text, for example). We all know that Word passwords, for example,
can be easily broken, but many people are honest and/or not bothered, so the
chance of misdirecting a document containing confidential material to
someone able and willing to crack the password must be significantly lower
than the chance of misdirection on its own, and may therefore reach the
required standard of 'appropriate' security.
