Further thought:
If you had not given the staff member concerned adequate training,
supervision, etc, to avoid this situation then you would already have failed
in your duty of care. If you had given them the training, and they still
recorded an inappropriate comment, they are largely the author of their own
misfortune, and have to accept the consequences.
I always appeal partly to self-interest in the data quality part of my
training sessions, and remind people that they would do well to consider the
Data Subject's possible reaction if inappropriate material is recorded.
