From what I heard on the Radio it sounds like it was an "envelope
stuffing" error. But there is probably something printed on the
statements which the stuffing machine uses to tell when one bill has
ended and another begins. Or if it's all one big machine, then the
layman's term "printing" in this case means "printing and stuffing".
Alternatively, we could ask: of the pages sent to the wrong customers,
were copies also sent to the right customers, or have they been "lost"
in that sense.
