I would certainly insist on 'proof' of who they are and this cannot be done without a full name and address (I request copies of documentation birth certificate, drivers license etc). If you have CCTV footage of them, then ensure they supply a photo so you can Identify them. I would also want a 'real' signature on a/the 'form'..
We can provide the data in an intelligible form (Section 7c of DPA). I would suggest you cannot guarantee who you are sending the data to if it is done by email and therefore I would send hard copies in post or ask them to collect. Normal email is certainly not a secure method of transferring personal data anyway. Certainly request the £10 if appropriate in circumstances.
I would write back asking for the above and ask if they have anything they specifically require (rather than all their data) as this will help cut the time it will take to respond.

I have received a SAR by email for 'everything about me'. Can I insist on a full name and address ? How can I satisfy myself as to the persons identity with just a name and email address ?
Our procedure is to write asking for completion of our form, payment and in this case, more information to find any data. Can they insist on our emailing any data we may have ?
