decision ‘product’ eligibility there are appeal forms available here:

(Use the appeal template). Note these forms are for students contesting the
way in which the SLC have interpreted the law. Complaints about levels of
service go elsewhere.
This form can be used to appeal if the student has been told they are not
eligible for DSA.
This form can be used to appeal recommendations in the DSA needs
assessment report that have been refused. The form can be used for ‘first
level’ appeal (which is explained in the leaflet),
