We'd do some preliminary screening here, esp. for dyspraxia as well as dyslexia as you appear to have little evidence either for or against dyslexia.
Quite apart from DSA our HEI (and many that I know of) would not approve specific provision for assessment without more robust evidence.

Dear All,
I have a student who had a statement whilst at school as he has a specific learning difficulty, not dyslexia. He has provided me with a one page document with no heading and no name, signature or contact details.
He wants to apply for DSA so will need further evidence. Our dyslexia tutor has asked me to find out what other institutions do in these cases. Would you normally just send these through your EPs etc, assuming of course that you have briefed them and that they are happy to see them?
