I thought I might write and reply to this one, we are starting off this process although I wouldn't say that we have it entirely sorted at the moment.
Our PEEPS are drawn up between the student, the Head of Health and Safety and myself (actually with very little input from me). Where necessary we might need to obtain further information from other staff particularly residential managers. Either the head of Health and Safety or myself would gather this.
Generally, we've decided which students need a PEEP that is me. We basically offer PEEPs to all students with physical or sensory disabilities and then anything else that crops up that we might be concerned about or that someone raises a concern e.g. epilepsy. We offer the PEEPS but we don't enforce it that students have them. I have been told by an external advisor to the Uni that we should enforce it, but being disabled myself don't feel that we have the right to say, you have to have a PEEP or you can't come here. This is usually because PEEPs are paper documents that tick boxes but will not be effective in every situation and I just don't see that we can insist that someone has one.
We have a generic PEEP form but we don't draw up generic templates for certain disabilities.
The head of Health and Safety signs off the PEEP.
We may try to complete the PEEP prior to arrival but usually it happens once the student is on campus.
In terms of reviewing the PEEPs, we still trying to sort out the initial system, let alone reviewing them!!!
