Except that one would hope you either have absence or you don't,
likewise discipline.
The absence of a disciplinary record should not indicate that the person
opted out of being disciplined.

The coding frame should specify these.
Of course, an absence of a record would be a "non-response". There can

many reasons for this. These can include that the person declined to
respond
and that they were not found.
