Have you notified of this type of disclosure at the time that you took
the information from the witnesses? If not then I would say yes, they
need asking again.

I have a request from one of our students to provide the names and
addresses of witnesses, and the full name of the person who attacked her
earlier this year so she can complete the compensation form referenced
above.
Has anyone dealt with one of these before? Is there any problem with
providing this information? Should we ask the witnesses first?
