Yes A student in his 3rd year. Payment of the invoice was refused on these grounds and it was suggested that we charge the student! The student had to write strongly to the LEA concerned. They paid up in the end.

Hello all
Has anyone had problems with LEAs not agreeing to pay for NMH support for
DSA students in subsequent years of support on the grounds that they have
not completed and returned their annual DSA1 form?
I wonder if we are going to be expected to check up annually that all our
returning students have had that year's DSA agreed before we allow them to
take up support, even if it has been recommended, agreed and paid for in
previous years?
