Can anyone tell me whether they have come across this scenario before please.
I have a student who has started an undergrad course here and is applying for DSA. The student has previously been awarded DSA on a (undergrad) course studied 3 years ago 2003/04 - she only completed 2 of the 3 years due to personal circumstances. She no longer has a copy of her Assessment of Study Needs and requested a copy from her LEA. They have stated that as any files that are over 2 years old are stored off site the student will have to pay a charge of between £20 and £30 to retrieve the file.
The centre that carried out the assessment of needs no longer has a copy either.
I discussed this at some length with the LEA who stated that students should take more responsibility with their reports as they cost a lot of money etc.
I queried the student's application for DSA and was informed that they would only agree to a top up assessment or review of the original assessment with updated recommendations (understandable) but in order for me to arrange that I would need a copy of the original report and the only way of getting it is from the student who would therefore have to pay the charge for retrieving the report.
Has anyone come across these type of charges before?
