I am thinking of database treatment as a general item.
If there is a rationale for retaining gender or other medical history that
can be justified (Adequate and not excessive data) then it may be held. If
there is no rationale it should not.
Gender Reassignment is a surgical process. So, if it is relevant to hold on
file other major surgical processes then so is it relevant to record this.
But not, I think, against the Gender field in the database.

Really? Changing gender may well have health issues which an insurance
company could legitimately take an interest in. Changing nationality (short
of moving to Iraq) wouldn't be quite the same thing.
