"simple" being the key word there. If spam detection could be done with
simple rules, there wouldn't be a spam issue in the first place. So the
simple rules are likely to cause more harm than good.

I really see this as an issue for the ad-hoc "send this thing to lots of
people", rather than regulated, repeated sends over a period of time.
People seem unlikely to bother to prime any sort of mailing list
application for an ad-hoc mailout, in my experience.
For the repeated sends, I'd certainly agree a mailing list application
(whether PC-based or mail-server-based) is the preferred solution.
Unfortunately, end users generally don't think about it that much. At
least if we encourage them to do the best they can with the tools they
have to hand, we minimise the problems (and the situation that started
this thread in the first place), then we can concentrate on targetting
those users with more particular needs.
I have a (high) limit on the length of a To: or Cc: header in a message,
effectively constraining the number of recipients. I have often
threatened to lower this brutally; maybe I should just go ahead and do it.
I don't recall if I have a limit on total recipients (which would include
Bcc: addresses too) for any particular message.
