I think one of the questions is: "Have you ever had a Visa revoked".
Except many travellers will have had "Indefinite" Visas many years ago,
which were all automatically revoked on your next entry to the USA when
they brought in the Visa Waiver scheme.
That's when you learn that "Indefinite" means "for a length of time we
are unable to quantify right now", rather than "for ever".
I did ask, and they said this episode doesn't qualify for a "yes" tick
on the form.
