There are many customer-facing roles where dealing with the pent-up
frustration of those who've got a raw deal requires more than you'd
expect from the average office worker. Where we seem to be going wrong
is in failing to defuse the frustration earlier, and then sending
average office workers in to deal with it.

That has never been in dispute.

It is apparently common for letters from organisations to bear "false"
(made up) names. Or just be signed by the organisation.

Yes, chainsaw wielding is a bit extreme. What makes someone resort to
such measures, and how can we work harder to resolve the issues before
it gets to that stage?
