I don't see the need for a separate form. You should be asking ALL
students about medical conditions etc. and advising of terrain,
physical challenges etc.
It might be sensible to ask students to advise you of any access
requirements that they feel you do not already know of that might be
relevant to the particular circumstances of a field trip. But you
should describe the paths, walks, activities for ALL students.
