I see that the IC's new website is now a triumph of design over
functionality. Even with your excellent instructions it took me 4 goes to
find the thing!

In terms of transparency of government this is opaque, to say the least!
Because I couldn't find this first time round I suggested to Julie that she
email them and tell them what she wanted to do and ask them how to do it.
Frankly, after the saga of finding the form, I stand by that private advice
in public!

It used to be a lot easier to find so I'll post this reply to the list in
case anyone else needs it.
It's now under the "Notification Publications" section in the menu on the
left. On that page there is a link to a "Form to Alter or Remove a Register
Entry".
