They actually get the original form I believe, which we then complete.

I'm fairly numpty at this stuff (time for some training) but from my reading of this:

This the Govt's Dept of Constitutional Affairs webpage on the Data Protection Act.
It would appear that the request is exempt as it's purpose is for preventing fraud i.e. detecting crime.
As you say it's not personal info as such, more corporate info.
It would be more simple if the NHS form included a statement saying that signing this form indicates that you agree to allow the release of information to verify your status to receive a free eye test.
