Hi James,
We loan equipment out to students and have a number of forms that students complete beforehand. I am presently re-drafting the policy for loaning equipment as we had an incident whereby a student had the laptop stolen. We are now only lending equipment if the student has insurance to cover loss or damage, if they don't we have a waiver form/letter that basically states that if it is lost or damaged the student is liable to pay for a replacement or repair costs. I am more than happy to send by post copies of the forms we have but bear in mind they are likely to change.
