I would totally agree with Gillian's point. As the student is using a tape
recorder as an alternative means of taking notes, there shouldn't have to be
a special form to fill in. Just as a matter of good practice, I do think it
might be worth approaching the lecturers first so that they are aware the
student is taping for access purposes. Having used a tape recorder in
lectures myself, I always took that line of approach while at university
myself and have never had any problems with it.

The student should not have to do any such thing, giving a lecture is
a form of communication, taking written notes is another, tape
recording is another.
A couple of years ago we inserted a clause into our regulations
which which states that: " Students with disabilities may be
permitted to use adaptive technology such as dictaphones and
recorders during lectures, however the intellectual property remains
with the lecturer. . ." Subsequently the rare arguments against
using any form of technology are generally very short.
When I am writing to academics on behalf of individuals I will ask
them to encourage the student to integrate assistive technology into
their studies. Which is a very broad brush statement but reinforces
the point.

Hello all
Does anyone have a good example of a form that students can sign
to
state that by recording the lecture, they will only be using the
information for personal, study-related purposes - or something like
that?
