As far as I'm concerned, there's no such thing as levels of dyslexia.
There's a student here who has a reading age of 6, who can't even sign his own name, and he is getting 2.1s for all his work - early identification, excellent support throughout education, and amazing use of technology means that despite the scores on his psychologist's assessment, he cannot be regarded as severely dyslexic.

I was wondering if anyone has been asked to indicate the levels of dyslexia
to academics and what support should be provided to respond to the level?
I have some concerns about this and your experiences and thoughts would be
