George
I assume that this monster-sized questionnaire was relating to you supplying
the institution with equipment for institutional use, rather than for
supplying their students with DSA funded equipment?
Richard.

Subject: Contractor/Supplier Questionnaire
I dare say that are a mere profiteering, money grabbing supplier of
adaptive technology, I may not get my sympathy, but here goes anyway.
I've just received a letter from a College, (which shall remain
nameless, for now!) telling me that if I do not complete their enclosed
questionnaire, we will be removed from their "Approved Suppliers' List".
It's 23 pages in total.
Part A, Company Information. 17 Questions.
Part B, Technical Information. 34 Questions.
Part C, Financial Information. 28 Questions.
Part D, Equal Opportunities Policy. 10 Questions.
Part E, Health and Safety Policy. 10 Questions.
Part F, Business Policy. 6 Questions.
Part G, Declaration. (Requires to be signed and
dated)
If I'm not mistaken, that totals 105 questions to be answered or
information to be supplied.
They want Banker's References, Accountant's References, THREE years of
Balance Sheets, P&L Accounts, Full notes to the Accounts, Director's
Report/Auditors Report, etc., etc., etc..
And yes, they are serious about us being struck off their list if we
don't complete it.
FOR SIXTY POUNDS WORTH OF BRAILLE PAPER!
I sure hope they never need to buy a box of toilet rolls from their
local Office World!
All the best,
