I doubt if such a thing exists. If I can put my pennyworth in as a
librarian, I note the 2001 edition of the
Charities Digest
lists a considerable number of outfits under the heading Disability. A
very rough count in the index is 600 plus. Not all will be relevant, of
course. Here at the Lanchester Library in Coventry we have a number of
similar general directories of varying currency and usefulness.
I'm sure your library ought to have similar material.
If you check out one or two of the sites here

you might find something of help - although it is really a page
constructed on the basis of what I happen to have book marked rather
than the result of a proper analysis of what is out there on the web.
There are lots of gaps.
