Firstly apologies for clogging up the list with emails, by accidentally pressing 'send' and then trying to recall my unfinished message.

Dear Rosemary (and all),
Our institution already has a cover sheet of the kind you describe in use and I am not aware of any difficulties (I may be wrong!) As I understand it most sheets in use also have a box for the student to tick to say whether they have a Learning Contract, (i.e.. student specific learning requirements negotiated between the institution and the disabled student). This may be one strategy to make explicit to the marker that any acknowledged help is regarded as acceptable by the University.
The other thing is I would be concerned by any suggestion that dyslexic support means an essay/ coursework is 'no longer all the student's own work'. If the wording of the statement is specific to plagiarism should it really be necessary for a dyslexic student to be made to declare they have met with a dyslexia tutor, I'm not sure?

We're having a discussion locally about how to word a statement all students
must sign on the cover sheet of their coursework before handing it in. A
recently revised statement asks students to declare that the coursework is all
their own work and to acknowledge any assistance which has been given. The aim
of the statement is mainly to deal with possible plagiarism, but there are
concerns that the need to acknowledge assistance may disadvantage dyslexic
students.
Have you had this discussion in your institution? What kind of declaration, if
any, are students asked to make when they hand in coursework?
