This is the form we use (from the other side of the coin as it were). Must
admit we don't currently give students the option of not signing up to all
of it at the moment, tho I have had the odd lecture on disempowering
students and may have to look at this when students stop ringing up and
asking for money all the time.

Does anyone use a letter / form that they ask students to sign in order
to comply with the Data Protection Act (DPA)?
We currently ensure that students give us permission to disseminate any
required information to relevant departments / LEA's. However, we are
always looking to improve our system and it would be useful, if people were
willing, to share any forms / letters.
