regardless of the law, a patient is under stress and so is an accompanying person. I would view it as a common courtesy to explain the processing of data in such a manner as to give the patient reassurance at a time when they or their representative is in a state to understand it.

When a patient we are incapable of understanding much of what is going on. In an ambulance we travel in shock and as a parcel. In casualty and beyond we are treated in a stressful manner by virtue of being kept in ignorance of what is happening. We are thus unable to take information on board.

I have had recent personal ambulance and A&E experience. Odd, if the detailed history taken by ambulance staff was handed to the hospital that the A&E team took a fresh total history form the very start.

I am currently reviewing all forms used to collect personal data within the
Trust and I have a small (hopefully) query.

The ambulance service complete a detailed form for every emergency patient
carried, showing patient details, medical condition, medical interventions
carried out etc. A copy of this form is then retained by the Trust and the
other copy given to the hospital to then be filed on the patients notes.
Unless an SAR is received at a later date, the patient does not actually see
a copy of this form. The form itself does not have a fair processing notice
on there (there isn't room!!). However, in all the ambulances there is a
fair processing notice placed in a prominent position so the patient, or the
person accompanying the patient, can see why the information is required,
what may be done with it etc. There are also information leaflets available
in the hospitals for patients to help themselves to.

Finally, my question is, will this suffice to comply with the Act or should
there be a notice on the patient report form also?
