Hi,
I am about to create a form (electronic) for internal notification (like the
one at the commissionners office but simpler). Looking for any pointers in
case I go down the wrong track.
I am interested to know what activities are taking place in the University,
who, where creates databases (paper or electronic), what do they do with the
data (esp. disclosures) and so on. Has anyone done anything similar that I
could have a look at.

PS. I am aware it is not an "audit" as implied by the ... audit guidelines
.. at the commissionner's site (can't access it right now as they appear to
have tech. problems and I can't find my copy).
